How does the enrollment work? |
|
It is simple, the Patriot Health marketing department will design & develop
enrollment brochures and postcards which can be mailed directly to the homes of
those in an eligible class. The enrollment information will direct interested parties
to call the Patriot Health toll free 800 enrollment center. Our knowledgeable enrollment
staff will assist the interested party in selecting a plan that feeds his or her
needs. This process is supported by the Patriot Health online enrollment center.
This process is available for annual as well as ongoing new hire enrollment. |

Must members go to an in network PPO provider? |
No.
While we always select a PPO network with the coverage to meet the geographic scope
of our clients
members can visit any doctor or hospital. However, if a member visits an In Network
Provider they will benefit
from the valuable and deeply negotiated discounts. We offer a multiple of PPO networks
which include Beech
Street, Multi Plan and Galaxy. |

How many plan designs can we offer? |
|
Traditionally 2 plans can be offered to any group. |

Will members receive a fulfillment package and membership cards? |
Yes. A fulfillment kit with 2 membership cards will be mailed to the members designated
address within 72
hours of receipt of enrollment |

How are claims processed? |
If the member visits an in network provider then, after the visit, the health care
provider will submit the
claim to the Patriot Health Claims center. The Patriot Health Claims Center will
re price the claim. The
healthcare provider will then balance bill the member for the difference between
the re priced claim and
benefit amount.
If the members visits an out of network provider then the member must pay for the
services at the time of the
visit and submit the claims form (with assistance of the Patriot Health Customer
Care Consultant) to the Patriot Health Claims Center. Claims are traditionally paid
within 14 days of submission. |

Is Contribution from an employer or group required? |
Contribution is optional. However, participation in plans increase dramatically
when some percentage of
contribution is extended. |
